The Village at Heritage, as we were originally known, came to be after two decades of community outreach, planning and construction. Specifically created to fill a demand for high-quality senior care, we opened our doors in 2000 to residents in Monroeville, Fort Wayne and surrounding towns. Based on family and community needs, we constructed our outpatient rehabilitation clinic in 2003 to include physical, occupational and speech therapy services, and we built Heritage Hall to accommodate family gatherings and community events. Although we changed our name to Adams Heritage in 2012, we continue to provide the same outstanding care and service today that our reputation is based on. We are still owned and operated by Adams Memorial Hospital, a member of Adams Health Network, so our ties to the community remain firm.
Adams Memorial Hospital, located in Decatur, Ind., is a 25-bed critical access hospital offering compassionate care and state-of-the-art technology. Founded in 1923, it has brought health and wellness to the community for almost 100 years. Adams Memorial offers over 50 services and continues to grow to meet the needs of our community. Specialty services include orthopedics, OB/GYN services, neurology, pulmonology and primary care.
Adams Health Network expanded its primary care services in 2022 by opening a new physician’s office in Monroeville. Called Adams Medical Group Monroeville, the office is located at 504 W. South Street. Dr. Maria Snyder is the primary care provider at the Monroeville office, and she is committed to providing excellent care to all patients and listening to their needs to provide the best possible outcome.
