We focus on a multidisciplinary approach consisting of nurses and physical, occupational and speech therapists. Our goal is to help patients achieve independence and a better quality of life. This type of care usually lasts a few weeks but can continue for a few months depending on the patient’s needs and recovery rate. Our team will see to your health care needs while our therapy team customizes a plan to get you back to what you love. Our therapy services are an effective and safe way to help patients regain their balance, strength, coordination and function.

 

How Do You Qualify for Short-Term Rehabilitation?

Our care navigator can help determine if a patient qualifies for a short-term rehab stay. Short-term rehabilitation, sometimes referred to as swing bed, is covered by Medicare and Medicaid as well as many private insurance plans. Medicare reimbursement requires a three-day qualifying stay in any critical access hospital (such as Adams Memorial Hospital) or acute care hospital prior to admission to short-term rehab or a swing bed program. Other insurance plans may allow short-term rehab (or swing bed) status, but authorization must be confirmed prior to admission. Additionally, a screening must be completely by a case manager or care navigator to assure patients are eligible to qualify.

Short-term rehabilitation is ideal for a variety of patient types:

  • Recovery from major surgery, joint replacement, vascular or abdominal surgery
  • Wound care
  • Rehabilitation after a stroke
  • IV medication therapy or nutritional therapy
  • Strength improvement after illness or prolonged hospitalization
  • Need for intensive physical therapy, occupational therapy and speech therapy

Our rehab team is employed by Adams Health Network and they work together to provide patients with a smooth transition to a range of settings, including back to home, home care and long-term care centers. Our therapists are on-site six days a week for the convenience of our community. We offer state-of-the-art therapy equipment for a safe transition to home or the next level of care.

Our rehab team uses a range of techniques that encourage movement while reducing pain, renewing functions and preventing future disability. Our outstanding therapy activities include:

Transitioning Back to Home

Along their road to recovery, residents enjoy three square meals per day, complimentary cable and Wi-Fi, laundry and housekeeping services and engaging activities. Both semi-private and private rooms are available with an in-room bath and walk-in shower as well as space for personal items.
